If you have any questions about your coverage and benefits, please contact your insurance carrier prior to scheduling in our office. Find out the specifics about what your plan covers and does not coverage. Ask about any exclusions or limitations to your plan.
Deductibles, Co-Payments, Co-Insurance
Fees will be collected at the time of service unless arrangements have been made in advance. Our office will submit an insurance claim on your behalf.
Non-Covered Services or Private Pay
